Why Does Text Neck Cause Upper Back Pain?

Short answer
Upper Back Pain is a common presentation of Text Neck because forward head posture makes the upper-back and shoulder-blade muscles work for long periods.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common area
- Between the shoulder blades or across the upper spine
- Useful change
- Break up stillness with frequent comfortable movement
The neck and shoulder-blade link
Looking down at a device often brings the head forward and the shoulders into a rounded position. Muscles between the shoulder blades and along the upper spine then help hold the shoulder girdle steady while the neck supports the head. Staying still can leave these tissues tired, tight, or tender, particularly between the shoulder blades. Upper-back discomfort may also come from the ribs, spine, shoulder, or other causes, so the pattern and examination are more useful than the location alone.
Relieve sustained loading
Place the screen high enough that you can look ahead more comfortably, and rest your elbows when using a handheld device. Alternate sitting, standing, and walking tasks, with regular shoulder rolls and gentle upper-back movement. Avoid forcing a stretch into pain. Building endurance in the shoulder-blade and trunk muscles may help when introduced gradually by a physiotherapist. A supportive work setup should allow movement rather than making you hold one ideal position all day.
When to get checked
Get urgent care for upper-back pain after major trauma, or with chest pressure, breathlessness, fainting, sudden weakness, or severe illness. Arrange prompt assessment for pain with arm weakness, persistent numbness, fever, or unexplained worsening. A routine appointment is sensible when discomfort limits work or sleep, keeps recurring, or does not settle after reducing prolonged screen positions.
Questions for your appointment
Which tissues may be involved in my upper-back pain? Can you review my screen setup and shoulder movement? What exercises should I start, and what symptoms mean I should stop?

Questions people ask
Why does pain occur between the shoulder blades?
Rounded shoulders and a forward head can keep the muscles around the shoulder blades working continuously, which may lead to fatigue and tenderness.
Can better posture fix it immediately?
Symptoms may take time to settle; varying positions, reducing strain, and gradual conditioning are usually more practical than holding a corrected posture rigidly.
