What does testicular pain feel like with inguinal hernia?

An inguinal hernia occurs when tissue pushes through a weak area in the lower abdominal wall. Pressure near the groin can irritate nearby nerves and structures leading toward the testicle. You may notice a bulge in the groin, heaviness, tenderness, or discomfort that improves when lying down. Testicular pain can also have other causes, so the sensation alone cannot identify the cause.

Arrange a clinical assessment if pain continues, returns, or comes with a groin lump. Until you are assessed, avoid heavy lifting and activities that clearly increase the pain, and use supportive underwear if it feels comfortable. Do not press forcefully on a painful or firm bulge. A clinician may examine the groin and scrotum and decide whether imaging or surgical advice is needed.

Seek urgent medical care for sudden or severe testicular pain, a testicle that becomes swollen or sits differently, vomiting, fever, or a groin bulge that is very painful, hard, discoloured, or cannot be gently pushed back. These features can signal a time-sensitive problem affecting the testicle or trapped hernia tissue.
