What should you know about under eye spots not milia?

Milia are small keratin-filled cysts, but an under-eye spot may instead be a blocked pore, an inflamed bump, a benign growth, a pigment change, or irritation. Whiteheads tend to relate to a blocked follicle, while other skin bumps may be firm, flat, yellowish, red, or related to a nearby eyelid gland. Colour and texture alone cannot confirm the cause.

A clinician will consider how long the spot has been present, whether it is growing, itchy, painful, bleeding, or recurrent, and whether there are multiple similar lesions. Do not treat a spot as milia simply because it is pale or sits near the eye.

Take a clear photograph for comparison and use a gentle cleanser without scrubbing. Avoid needles, squeezing, abrasive products, and non-prescribed acids near the eyelid. A clinician may recommend observation, a topical retinoid for a suitable diagnosis, or removal such as curettage and cautery when appropriate; treatment must account for the delicate eye area.

Tell the clinician about cosmetics, new skincare, allergies, medicines, and previous attempts to remove the lesion. Follow directions closely because irritation near the eye can be more troublesome than the original spot.

Arrange a dermatology review for a spot that persists, changes shape or colour, crusts, bleeds, becomes painful, or keeps returning. Seek prompt help if swelling affects the eye, vision changes, or the area becomes hot, very red, or associated with fever.
