Stomach pain during sex: what to know

Pain may be felt in the lower abdomen or deep in the pelvis rather than in the stomach itself. It can begin with penetration, become noticeable with deeper thrusting, or continue after sex. New, repeated, or worsening pain deserves a clinical assessment, especially when it affects intimacy or daily activities.

Deep pain can be associated with endometriosis, an ovarian endometrioma, pelvic inflammatory disease, or another pelvic condition. A person with endometriosis may notice pain that is worse during or after sex, while pelvic inflammatory disease may also cause unusual discharge, fever, or lower abdominal tenderness. These patterns do not identify the cause on their own.

Pain at the vaginal entrance may relate to vulvodynia, skin inflammation such as lichen sclerosus, dryness, or irritation. Genitourinary Syndrome of Menopause can cause vaginal dryness and tissue changes that make sexual activity uncomfortable. Muscle tightening, insufficient lubrication, or a position that puts pressure on a tender area can also contribute.

Pause sexual activity if it causes pain, and do not push through it. Note where the pain is, whether it is linked to penetration or depth, when it starts, how long it lasts, and whether it follows your menstrual cycle. Record bleeding, discharge, urinary symptoms, bowel symptoms, fever, or nausea as well.

Gentler positions, slower activity, and a suitable lubricant may reduce friction or depth-related discomfort. Arrange an appointment with a gynaecologist or primary-care clinician if the pain returns, is new, or has no clear short-lived explanation. Evaluation may include a symptom history, examination, pregnancy testing, infection testing, or imaging depending on the pattern.

Seek urgent medical care for severe or rapidly worsening pain, fainting, marked dizziness, heavy bleeding, fever with pelvic pain, repeated vomiting, or a rigid or very tender abdomen. Get prompt assessment for pain with a missed period or possible pregnancy, particularly if the pain is one-sided or accompanied by bleeding.

Contact a clinician soon for pain that keeps returning, bleeding after sex, new discharge or odour, pain when urinating, sores or skin changes, or pain that is limiting sexual activity. These symptoms can point to an infection, tissue condition, or pelvic disorder that needs targeted treatment.

Ask: Could the location and timing of my pain suggest a pelvic, vaginal, urinary, bowel, or muscle-related cause? Do I need tests for pregnancy or infection? Could my menstrual pattern or menopause-related changes be relevant? What symptoms should make me seek urgent care? If endometriosis is suspected, what treatment options are appropriate before considering laparoscopic excision?
