Smoking and ectopic pregnancy: what to know

An ectopic pregnancy develops outside the main cavity of the uterus, often in a fallopian tube. Tobacco chemicals can affect the tiny movements that help an embryo travel through the tube, which may make implantation in the tube more likely. Smoking is one factor among several; previous pelvic inflammatory disease, tubal surgery, and a prior ectopic pregnancy can also matter. Smoking does not make an ectopic pregnancy certain, and stopping now can still support overall reproductive and pregnancy health.

Ectopic pregnancy may begin with a missed period, positive pregnancy test, light bleeding, or pelvic pain. Symptoms can be mild at first, so do not wait for severe pain before seeking advice.

If pregnancy is possible and you smoke, arrange early pregnancy care and mention your smoking history. Ask for support to stop smoking; counselling, nicotine-replacement options, and medicines need individual review in pregnancy. If you have pain or bleeding, contact an urgent maternity or medical service and explain that pregnancy is possible. Assessment may include pregnancy hormone tests and an ultrasound.

Seek emergency help for one-sided pelvic or abdominal pain, shoulder-tip pain, dizziness, fainting, collapse, or heavy bleeding. These symptoms can indicate internal bleeding from a ruptured ectopic pregnancy. Do not drive yourself if you are faint or severely unwell.
