Recognising the pattern

Look for smooth or shiny skin where follicle openings are hard to see, especially if the area is expanding. Itch, burning, tenderness, redness, scale, pustules, or crusting can accompany active disease. By contrast, alopecia areata may produce smooth patches without scarring, and male or female pattern hair loss tends to show patterned thinning. Scalp ringworm, telogen effluvium, and traction alopecia each have different clues and treatments.

Finasteride treatment is not a universal answer and is only suitable in particular circumstances.

Seek dermatology care soon when loss is rapidly spreading, the scalp is painful or inflamed, or follicle openings appear to be disappearing. Same-day help is appropriate for fever, pus, rapidly increasing warmth, severe swelling, or intense pain. Early attention matters because scar tissue cannot be converted back into a functioning follicle.
