What should you know about ringworm in scalp hair loss?

Scalp ringworm is a fungal infection of the scalp and hair shafts. It may produce round or irregular patches of broken hairs, itching, flaking, tenderness, or swollen areas. Hair loss can also result from alopecia areata, pattern hair loss, telogen effluvium, or traction alopecia, so the presence of hair loss does not by itself identify the cause.

A clinician may inspect the scalp and send hair or scale for testing. This distinction matters because untreated infection can inflame the scalp and spread to close contacts, while different forms of hair loss require different care.

Arrange an assessment rather than relying on an over-the-counter rash cream. Scalp ringworm commonly needs an oral antifungal prescribed for the individual; a medicated shampoo may be added to reduce spread but does not replace the main treatment. Do not share combs, hats, pillows, towels, or hair tools while evaluation is underway.

Wash or replace items that touch the scalp according to clinical advice, and ask about household members and pets. Complete the prescribed course even if itching and shedding improve early. Hair often regrows after the infection and inflammation settle, but severe inflammation can leave lasting change.

Seek prompt care for a painful boggy swelling, pus, marked tenderness, fever, rapidly enlarging bald patches, or swollen neck glands. Urgent assessment is also sensible when a child is affected, several household members have symptoms, or the person has reduced immune defenses. Avoid starting steroid cream without advice, because it can alter the appearance of a fungal infection.

Ask which test is needed, whether an oral antifungal is appropriate, how to reduce household spread, when school or sport is reasonable, and what signs would suggest a separate hair-loss condition.
