What it means

Gout develops when urate builds up in the body and forms crystals inside or around a joint. A flare can bring intense pain, warmth, tenderness, redness, and visible swelling. The big toe is a familiar site, but ankles, knees, and other joints can also be affected. An episode may settle and then return, so symptom-free periods do not necessarily mean the underlying tendency has gone away.

Chronic kidney disease can reduce urate removal, while obesity may add to the risk of high urate levels. A swollen or painful joint can have many causes, however; joint pain or knee swelling alone does not establish gout.

What to do next

Arrange a medical assessment if this is your first flare, if attacks recur, or if pain is limiting walking. A clinician may examine the joint and use blood tests or joint-fluid testing when needed. Treatment can address the current inflammation and, for recurring gout, lower urate over time. Take prescribed medicines as directed, keep the joint rested and supported during a flare, and drink fluids unless a clinician has advised a fluid restriction.

Record which joint was affected, how quickly symptoms appeared, possible food or alcohol triggers, and every medicine you take. Do not stop long-term treatment or start a supplement without checking its safety.

When to seek care

Seek urgent medical care for a hot, very swollen joint with fever, chills, feeling faint, or rapidly worsening illness, because a joint infection can resemble a gout flare. Get prompt help after an injury if the joint looks deformed or you cannot bear weight. Contact your clinician soon when flares are frequent, treatment is not helping, or a new joint becomes involved.
