What to know about refusing chemotherapy

Chemotherapy may be offered to destroy cancer cells, reduce the chance of cancer returning, slow disease, or relieve symptoms. The effect of declining it depends on the cancer type, stage, biology, and whether another treatment is available. Concerns about nausea, tiredness, hair changes, infection risk, travel, work, cost, or previous experiences are worth naming. You can ask for a second opinion or a treatment break discussion; asking questions does not commit you to treatment.

Ask the oncology team to explain the aim in plain language, what may happen with treatment and without it, and which alternatives could fit your priorities. Discuss medicines that prevent or ease side effects, practical support at home, and palliative care for comfort at any stage. If you decide to decline, agree on follow-up, symptom control, and who to contact. Tell the team before stopping a course already started, because some medicines need planned changes.

Seek urgent help for severe breathing trouble, chest pain, confusion, uncontrolled pain, heavy bleeding, or inability to keep fluids down. During or after chemotherapy, contact your team urgently for fever, chills, new shortness of breath, or sudden marked weakness. If you decline treatment, report new symptoms early so they can be assessed and treated.
