Period cramps around ovulation

Ovulation is when an ovary releases an egg. Some people notice a short-lived ache or sharp twinge on one side of the lower abdomen around that time. It may switch sides in different cycles and can occur with a small amount of spotting. This pattern is often called ovulation pain, but timing alone cannot confirm its cause.

Pain that lasts, returns outside the middle of the cycle, or comes with heavy bleeding may have another explanation. Endometriosis, adenomyosis, fibroids, an ovarian endometrioma, or pelvic inflammatory disease can cause pelvic pain at different points in the cycle.

Record the first day of each period, the day pain begins, its side and severity, associated bleeding or discharge, and how long it lasts. Rest, gentle movement, warmth, and an appropriate over-the-counter pain medicine may help. If episodes interfere with daily life, a clinician can discuss evaluation and options such as cycle control with the combined pill or hormonal coil.
