Perimenopause and bleeding after intercourse

Changing oestrogen levels can make vaginal tissue thinner and less well lubricated. Friction may then cause small surface breaks and light spotting, especially when sex feels dry or uncomfortable. Bleeding can also come from the cervix, an infection, a polyp, or changes that are unrelated to perimenopause. A single episode may be minor, but repeated bleeding, bleeding after menopause, or bleeding with pelvic pain deserves examination.

Keep track of when the bleeding occurs, its colour and amount, whether sex was painful, and where you are in your menstrual pattern. This helps a clinician distinguish bleeding after contact from an irregular period. Perimenopause commonly causes irregular periods, but that does not explain every episode of bleeding after sex.

Arrange a routine appointment with a gynaecologist or primary-care clinician, particularly if the bleeding happens again. Until then, use a water-based lubricant, pause if intercourse causes pain, and avoid inserting products that irritate the vagina. Do not assume that spotting is simply part of perimenopause; an examination may include looking at the vagina and cervix and discussing infection testing or cervical screening when appropriate.

Seek urgent care for heavy bleeding, faintness, severe or worsening pelvic pain, fever, or bleeding during pregnancy. Contact a clinician promptly if bleeding is recurrent, follows a long gap without periods, or occurs with unusual discharge or pain during urination.

Ask what might be causing the bleeding, whether dryness or an infection could be contributing, whether you need an examination or screening, and which symptoms should trigger urgent help.
