The treatment approach
Asthma causes sensitive, narrowed airways that can produce cough, wheezing, chest tightness or breathlessness. A reliever inhaler opens the airways during symptoms. A preventer may be prescribed regularly to reduce airway inflammation and lower the chance of flare-ups; it should not be stopped simply because the child feels well.
Symptoms can be worsened by viral infections, smoke, exercise, allergic rhinitis or house dust mite allergy. Treatment is adjusted to the child’s pattern, inhaler technique, age and response.
Making the plan work
Use a spacer when prescribed, demonstrate technique at every review, and keep reliever and written instructions available at school and during activities. Track night waking, exercise limitation, cough and reliever use. Reduce smoke exposure and follow the child’s agreed trigger plan. A nebuliser treatment may be used in urgent settings, but it is not automatically better than an inhaler with a spacer.
When to seek care
Use the urgent part of the action plan if breathing remains difficult, the child cannot speak or drink normally, the chest is pulling in, lips look blue or the reliever is not helping. Seek emergency care immediately for severe breathlessness, exhaustion, confusion or a child who is rapidly worsening.
