What should you know about par defect?

Short answer

A pars defect is a stress injury or small break in the pars portion of a spinal vertebra, often managed with activity changes and rehabilitation.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What it means

The pars is a narrow bridge between the joints at the back of a vertebra. Repeated bending, twisting, or loading can strain this area. Some people have no symptoms; others develop low-back pain that increases with extension, sport, or prolonged activity. A defect on one side or both sides can affect spinal stability, and a vertebra may sometimes shift forward. Examination and imaging help distinguish this problem from muscle pain, disc symptoms, or nerve irritation.

What you can do

Pause movements that reliably increase back pain, while avoiding prolonged bed rest unless specifically advised. A physiotherapist can build a graded programme for abdominal, back, hip, and hamstring control, then guide a safe return to work or sport. Exercise therapy is usually adjusted to symptoms and healing findings. Pain relief may be discussed with a clinician who knows your medical history. If pain persists, instability or nerve symptoms may prompt further imaging or a spine review; spinal fusion is reserved for selected cases rather than being automatic.

When to seek care

Book an assessment for persistent or recurring low-back pain, pain that limits sport or work, or symptoms after a fall. Seek urgent care for new leg weakness, loss of bladder or bowel control, numbness around the groin, fever with severe back pain, or rapidly worsening symptoms. These signs need evaluation because they may involve the nerves or another serious spinal problem.

Questions for your doctor

Where is the defect, and is it on one side or both? Is there any vertebral slippage or nerve compression? Which activities should I stop temporarily? What rehabilitation milestones should guide return to sport? When would further imaging or surgery be considered?

Questions people ask

Does every pars defect cause pain?

No. Some are found without symptoms, while others cause activity-related back pain or contribute to spinal movement problems.

Can physiotherapy help?

A tailored programme can improve trunk and hip control and support a gradual return to activity when appropriate.
