Why the eye area can look different

The skin around the eyes is thin and can look lighter or more shadowed depending on lighting, sleep, hydration and natural complexion. If the inner lower eyelids, lips, gums or face also look pale, the change may be more general. Anaemia, including iron deficiency anaemia, can cause pallor and fatigue. Vitamin B12 deficiency, sickle cell disease and thalassaemia are other blood-related conditions that may be considered when the wider history fits. Under-eye appearance alone cannot identify the cause.

What you can check

Look in natural daylight without makeup and compare both eyes with your usual appearance. Check whether the inner eyelids, lips and palms have changed too. Note sleep quality, recent illness, diet, heavy periods, tiredness, dizziness, breathlessness, headaches or a racing heartbeat. If the change is persistent or widespread, arrange an examination; blood tests may be useful. Gentle skin care and rest may improve a temporary appearance, but do not use iron without medical advice.

When to seek care

Book a medical review if paleness under the eyes persists, is accompanied by general pallor, or comes with ongoing fatigue, weakness, dizziness or breathlessness. Seek urgent help for fainting, chest pain, severe breathing trouble, confusion, blue or grey lips, sudden collapse or significant bleeding. One-sided swelling, pain, redness or vision change around an eye also needs prompt assessment.
