What should you know about lupus and hair loss?

Hair loss with lupus may be diffuse shedding during active illness, inflammation or physical stress, or loss from a scaly, inflamed scalp lesion that can scar. Some people also notice breakage, changes in texture or thinning during recovery from a flare. Medicines, thyroid problems, iron deficiency, hormonal pattern hair loss, alopecia areata, scalp ringworm and tight hairstyles can produce different patterns.

The timing matters: clinicians may ask when shedding began, whether it followed a flare or medicine change, and whether there is itching, scale, pain, redness or smooth bald patches. A scalp examination helps separate shedding from damage to the follicles.

Tell your rheumatology or dermatology clinician about new loss rather than stopping lupus medicine yourself. Bring a medicine list and note whether the scalp is sore, scaly or sensitive. Gentle washing, low-heat styling, loose hairstyles and avoiding chemical treatments can reduce breakage while the cause is assessed.

Treatment may target lupus activity, scalp inflammation, infection or a separate hair disorder. Some shedding improves after the trigger is controlled, while scarring changes need earlier attention because damaged follicles may not regrow hair.

Arrange prompt review for rapidly expanding bald areas, scalp pain, crusting, pus, marked redness, thick scale or shiny skin where follicles seem absent. Seek urgent help for lupus symptoms such as severe chest pain, major breathing difficulty, confusion or sudden weakness. Do not wait for hair loss to settle if it accompanies a significant flare.

Ask whether the pattern suggests shedding, inflammation, scarring or another hair condition; whether blood tests or a scalp sample are useful; and whether a medicine could be contributing. Discuss safe styling, treatment goals and how quickly the scalp should be reviewed if symptoms progress.
