Asthma and chest X-rays: what to know

A chest X-ray usually cannot confirm asthma by itself, because asthma is defined by variable airway narrowing rather than a distinctive X-ray appearance. The image may help look for another explanation of cough, wheeze, chest tightness or breathlessness. Clinicians combine it with your symptom pattern, examination and breathing tests.

Asthma symptoms can overlap with infection and other chest problems. An X-ray may be considered when symptoms are persistent, unusual, severe or not responding as expected, or when the clinician wants to assess another cause. A normal image does not rule out asthma. Allergic rhinitis and house dust mite allergy can be relevant when symptoms follow an allergic pattern, while chronic cough may need its own assessment.

Tell the imaging team if you might be pregnant and describe your symptoms and medicines to the clinician requesting the scan. Follow instructions about inhalers and attend any arranged breathing test. Keep a record of wheeze, cough, chest tightness, night waking, exercise triggers and relief from prescribed treatment. Use your asthma action plan, including your reliever, exactly as advised. A nebuliser treatment may be used in urgent settings when inhaled medicine needs supervised delivery.

Seek emergency help if breathing is severely difficult, you cannot speak comfortably, your lips look blue or grey, you become confused or faint, or your reliever is not helping according to your action plan. Rapidly worsening wheeze or chest tightness also needs urgent attention. Do not wait for an X-ray appointment during a serious flare.

Ask what the X-ray is intended to check, what a normal result would mean, whether spirometry or another breathing assessment is needed, how to adjust treatment during symptoms and which signs require emergency care.
