What should you know about large milia on the face?

Milia are small, firm, pearly or white cysts formed when keratin becomes trapped near the skin surface. A larger bump may feel harder and remain unchanged for a long time. They are not the same as acne whiteheads, although the two can look similar in photographs or under makeup.

A clinician checks the texture, colour and position of a facial bump before treatment. Diagnosis is usually based on its appearance, but an unusual, inflamed or changing lesion may need a closer review. The thin skin around the eyes and other delicate facial areas needs especially careful handling.

Do not squeeze, pierce or scrape a large facial milium. Those actions can cause inflammation, infection, scarring or lingering colour change. Use a gentle cleanser, avoid harsh scrubs and tell the clinician about retinoids, exfoliants, cosmetics and any skin condition affecting the area.

If removal is wanted, a dermatologist may open the surface with a sterile instrument and release the contents. Curettage and cautery may be considered for selected lesions. Topical retinoids can be useful in some treatment plans, but they may irritate facial skin and should be used only as directed, particularly near the eyes.

Book an assessment if a bump becomes red, tender, swollen, crusted, bleeds, changes quickly or does not resemble your other milia. Seek prompt care for spreading redness, pus, marked swelling or pain around the eye. A rapidly changing facial lesion should be examined rather than treated as a cosmetic spot.

Ask whether the bump is consistent with milia, which removal method suits its location, how to protect nearby skin, what irritation to expect, and how to care for the area afterwards. If you use eye makeup or active skincare, ask when it is reasonable to restart each product.
