What can be behind it

A change in position, vivid dreams, stress, alcohol, caffeine, fever or dehydration may make beats feel prominent at bedtime or on waking. Snoring and disrupted breathing can also fragment sleep and affect how the heartbeat feels. An abnormal rhythm such as Atrial Fibrillation is one possible explanation, but the sensation does not diagnose it.

Ask a bed partner whether they notice pauses in breathing, gasping or unusual movement. Record whether the episode wakes you, how long it lasts, and whether it causes Dizziness, breathlessness or chest discomfort. An Electrocardiogram can help capture the rhythm.

Steps that help

Sit up slowly if you wake with pounding or an uneven rhythm, and avoid standing quickly if you feel light-headed. Keep a sleep and symptom diary, including snoring, alcohol, caffeine, illness and medicines. Arrange medical review for repeated awakenings, daytime exhaustion or episodes that occur without an obvious trigger.

When it is urgent

Seek emergency care for chest pain, severe breathlessness, fainting, collapse or a sustained irregular rhythm with marked weakness. New one-sided weakness, confusion, speech difficulty or vision loss needs urgent help because it can be a sign of Stroke or Transient Ischaemic Attack. Do not wait for a routine appointment if these symptoms occur.
