I pee a little when I cough: what can help?

A cough briefly increases pressure inside the abdomen. If the pelvic floor and the valve at the bladder outlet cannot counter that pressure, a small amount of urine may escape. Pregnancy, childbirth, menopause, pelvic surgery, constipation and repeated coughing can affect this control.

The cough may come from a common cold, acute bronchitis, allergic rhinitis, asthma, acid reflux or another airway condition. A cough lasting a long time deserves assessment because treating its cause may reduce repeated pressure on the bladder.

Arrange an appointment if leakage is recurring, bothersome or changing. Keep a brief bladder and cough diary: record drinks, urgency, leaks, triggers, urine burning, fever and how long the cough has lasted. A clinician may ask about medicines, bowel habits, pregnancies and pelvic symptoms.

Pelvic floor muscle training can improve bladder control when taught and practised consistently. Avoid straining to pass urine, treat constipation, and do not sharply restrict fluids unless a clinician advises it. A pharmacist or doctor can help choose cough treatment based on the cause.

Seek prompt care for blood in the urine, burning with fever, severe lower abdominal or back pain, inability to pass urine, new leg weakness or numbness, or sudden loss of bladder control. For the cough, urgent help is needed with severe breathlessness, chest pain, blue lips, confusion or coughing blood.
