How regular periods fit with PCOS

PCOS is diagnosed from the overall pattern of ovulation, androgen-related features, and ovarian appearance rather than from cycle timing alone. Some people with PCOS have regular bleeding but still ovulate inconsistently. Others have acne, increased facial or body hair, scalp hair thinning, or difficulty getting pregnant while their periods appear predictable. Regular periods do not rule out PCOS, and PCOS cannot be diagnosed from an ultrasound image alone.

Planning your care

Ask your clinician to explain which findings support the diagnosis and whether another condition could mimic it. If pregnancy is your goal, discuss ovulation confirmation and fertility planning. If pregnancy is not desired, contraception can still be useful; the Combined Pill may help with bleeding or androgen-related symptoms for some people but does not treat every PCOS feature. A care plan may also include review of blood pressure, glucose regulation, sleep, mood, skin, and hair symptoms. Treatment should match your priorities rather than the label alone.

When to seek care

Arrange a review for new or worsening facial hair, acne, scalp hair loss, heavy bleeding, missed periods, periods that stop, or trouble getting pregnant. Seek urgent help for very heavy bleeding with weakness or faintness, severe pelvic pain, or pregnancy-related pain or bleeding. These symptoms need assessment even when your usual cycle is regular.
