How pool exercise supports the knee

Buoyancy can reduce how much body weight the knee carries, making walking, knee bends and leg lifts easier to practise. Water resistance still challenges the muscles as you move slowly in different directions. A programme may include supported walking, gentle knee flexion and extension, heel raises, balance work or stepping, selected after an examination. The exercises should reflect your range of motion, strength, swelling and confidence in water. They can complement land-based exercise therapy rather than replace it.

Making practice useful

Begin with the movements prescribed by your physiotherapist and use the pool rail or another support when instructed. Keep your knee aligned with your foot during bends and steps; avoid forcing a painful range. Start slowly, rest before your technique changes, and check whether swelling or walking is worse later. Tell the clinician about locking, giving way, recent surgery, an open wound, or difficulty getting in and out of the pool. A suitable hydrotherapy pool should have safe access and supervision.

When knee symptoms need review

Stop exercise and seek prompt advice for sudden severe pain, a new inability to bear weight, marked swelling after a twist, or a knee that becomes hot and red with fever. New calf swelling or pain, chest pain or breathlessness also needs urgent medical attention. Arrange review when the knee repeatedly locks, gives way, or loses movement despite following the plan.
