Heartburn and coughing up blood: what to know

Heartburn is a burning feeling caused when stomach contents move upwards, while coughing up blood means blood is brought up from the breathing tubes or lungs. Blood mixed with mucus, pink froth, or repeated red streaks should not be assumed to be reflux. Heartburn does not explain away coughing up blood; seek urgent medical assessment to identify where the blood is coming from.

A forceful cough can irritate the throat and leave a small streak, but bleeding can also occur with chest infection, bronchiectasis, pneumonia, tuberculosis, a blood clot in the lung, or lung cancer. These conditions present with coughing up blood; the symptom does not establish which condition is present. Reflux may cause cough or throat irritation, yet it should be considered separately from visible blood. Tell the clinician whether the blood is bright red, mixed with phlegm, or accompanied by breathlessness, fever, chest pain, night sweats, or weight change.

Arrange urgent medical evaluation today, even if the amount seems small or the heartburn is familiar. Do not smoke, exercise heavily, or take a new cough remedy to mask the symptom. Note when it began, how often it occurred, and any medicines that affect clotting. Reflux measures such as smaller meals and staying upright after eating may reduce burning, but they are not a substitute for checking the bleeding.

Call emergency services immediately for more than a trace of blood, breathing difficulty, severe chest or upper-back pain, faintness, blue lips, confusion, or a rapid deterioration. Emergency assessment is also needed when coughing blood follows a recent operation, long travel, immobilisation, or a leg that is swollen and painful. Do not drive yourself if you feel faint or short of breath.

Ask whether the blood appears to come from the lungs, throat, nose, or digestive tract; what examination or imaging is needed; whether your medicines could increase bleeding; and how to control heartburn safely while the cause is investigated.
