Hair loss with red spots on the scalp

Red spots may follow product irritation, folliculitis, scratching, or a skin condition. Scalp ringworm can cause scale, broken hairs, and tender areas; traction alopecia can irritate skin where hair is pulled. Alopecia areata more often causes smooth patches without surface scale, while pattern hair loss is generally gradual and not inflamed. These patterns overlap, so an examination is more useful than guessing from a photograph.

Pause any recently introduced dye, oil, styling product, or chemical treatment if it stings or worsens redness. Wash gently, avoid picking, and stop tight hairstyles until the scalp settles. Do not share combs, brushes, hats, or towels if infection is possible. A clinician may inspect hairs and skin, and can select treatment based on whether the cause is irritation, infection, or inflammation.

Seek prompt review if spots are painful, spreading, warm, draining, crusted, or accompanied by fever. Arrange a dermatology appointment for bald patches, broken hairs, heavy scale, or redness that persists after removing an irritant. Rapid change or scarring features should not be managed only with cosmetic products.

Ask whether follicles look infected or scarred, which products to stop, whether household contacts need advice, and how to prevent further hair damage while the scalp heals.
