What should you know about a fistulogram MRI scan?

A fistulogram MRI scan uses magnetic resonance images to show the course of an abnormal tunnel, its openings, and nearby tissues.

What the scan can show

A fistula is a narrow passage that connects two areas that should not be connected, often after inflammation, infection, surgery, or an injury. MRI creates detailed views without x-rays. Depending on the body area, a radiology team may use a contrast substance to make the tract easier to follow. The images can help distinguish the main channel from branches, fluid collections, and involvement of muscles or other organs. An MRI finding does not by itself establish why the fistula formed or whether it is cancer-related.

Preparing and taking part

Tell the imaging team about metal implants, a pacemaker, kidney problems, previous contrast reactions, pregnancy, or difficulty lying still. Remove metal items and follow the instructions about eating, drinking, and medicines. During the scan you lie on a movable table inside the scanner and need to keep still; the machine can be loud, so hearing protection is provided. You can usually speak with the team through an intercom. Ask how and when your result will be explained, and whether another test such as a biopsy is needed.

When to seek care promptly

Contact your clinical team promptly for increasing pain, redness, swelling, fever, pus, a new opening, or worsening drainage around the suspected fistula. Seek urgent help for heavy bleeding, fainting, confusion, severe weakness, trouble breathing, or sudden facial or throat swelling after contrast. These symptoms need assessment because infection, bleeding, or a contrast reaction can require treatment.
