Can ectopic pregnancy occur without bleeding?

An ectopic pregnancy develops outside the uterus, often in a fallopian tube. Bleeding from the pregnancy may remain inside the abdomen rather than passing through the vagina, or bleeding may not have started. Vaginal bleeding is therefore not required for an ectopic pregnancy to be present.

Possible symptoms include one-sided pelvic or abdominal pain, pain that reaches the shoulder, dizziness, fainting, or feeling unusually weak. Some people have mild or no symptoms early on. Pelvic inflammatory disease and previous tubal problems can raise concern, but an ectopic pregnancy can also occur without a known risk factor.

If you could be pregnant and have pelvic pain, take a pregnancy test and contact a clinician promptly, even if there is no bleeding. A clinician may arrange pregnancy hormone blood tests and a transvaginal ultrasound. Explain when pain began, whether it is one-sided, and whether you have had faintness or shoulder-tip pain.

Go to emergency care immediately for sudden or worsening pain, fainting, severe weakness, shoulder-tip pain, confusion, or heavy bleeding. These symptoms can indicate internal bleeding or rupture. Do not wait for vaginal bleeding to appear, and do not travel alone if you feel unsteady.

Ask whether the pregnancy location has been confirmed, whether repeat blood tests or imaging are needed, and which symptoms require an immediate return. If treatment is recommended, ask about medication, surgery such as salpingectomy, fertility considerations, and follow-up.
