Why symptoms may change
Pain from an ectopic pregnancy may be felt on one side of the lower abdomen or pelvis, or may spread to the shoulder if blood irritates the diaphragm. It can fluctuate as the pregnancy tissue and nearby structures change. Some people have little pain at first, while others develop pain quickly. Intermittent pelvic pain is not specific to ectopic pregnancy and can also come from bowel, urinary, ovarian, or inflammatory conditions, so the pattern alone cannot identify the cause.
What to do about intermittent pain
If you could be pregnant, arrange prompt medical assessment even when the pain has stopped. Record where the pain occurs, how long each episode lasts, whether it follows movement, and whether bleeding, dizziness, or shoulder pain occurs. A clinician may use an examination, blood tests, and transvaginal ultrasound. If an ectopic pregnancy is found, management can include close monitoring in selected situations, medicine, or surgery such as salpingectomy.
When the pattern is an emergency
Seek emergency help immediately for severe or worsening pain, fainting, collapse, shoulder-tip pain, heavy bleeding, marked dizziness, or weakness. Do not wait for another episode or assume that a quiet interval means the danger has passed. If you feel faint, lie down and call emergency services.
