Decidual cast and adenomyosis

A cast may look like a fleshy, mould-shaped piece and can cause sudden cramping as it passes. Hormonal changes, including those associated with hormonal contraception or pregnancy-related changes, may be relevant. Passing a cast does not by itself diagnose adenomyosis.

Adenomyosis affects the uterine wall and may present with heavy periods, painful periods, or pelvic pain. Symptoms can overlap with other gynaecological conditions, so the appearance of passed tissue is not enough to identify the cause. If possible, place the tissue in a clean container or take a clear photograph for a clinician.

Arrange medical assessment, especially after a first episode, repeated casts, severe cramps, unusual bleeding, or a possible pregnancy. A clinician may review contraception, pregnancy possibility, cycle timing, medicines, and previous uterine problems. Examination and imaging can help assess the uterus; tissue may sometimes be sent for laboratory review.

Treatment depends on the cause and your plans for pregnancy. Options for adenomyosis may include a Hormonal Coil or specialist procedures such as Uterine Artery Embolisation, Endometrial Ablation, or Hysterectomy. These choices have different fertility and recovery implications.

Seek urgent care for severe or worsening pelvic pain, heavy bleeding with weakness or dizziness, fever, fainting, a positive pregnancy test with pain or bleeding, or tissue that cannot be passed comfortably. Emergency assessment is also appropriate if you feel acutely unwell.
