Dark discharge during perimenopause

Dark discharge during perimenopause is often old blood from an irregular cycle, yet persistent, heavy, painful or unusual discharge needs assessment. As ovulation becomes less predictable, bleeding may arrive earlier, later, or after a gap; blood that leaves slowly can look brown or dark. Vaginal dryness can also make minor bleeding after sex more likely.

Perimenopause commonly brings [irregular periods](/symptoms/irregular-periods/), and dark discharge may appear at the beginning or end of bleeding. Consider whether it has a strong smell, grey or green colour, itching, burning, pelvic pain or pain during sex, because these features may point to infection or another cause. Track discharge, bleeding days, cycle gaps, pain, sexual activity and any new medicines or contraception. Use unscented products and avoid washing inside the vagina. If pregnancy is possible, test as advised by a clinician because fertility can continue during the transition. Book an appointment when the change persists, recurs, follows sex, or affects daily life; an examination can distinguish blood from infection-related discharge.

Seek urgent care for bleeding that is heavy, faintness, severe pain, fever, or bleeding with a possible pregnancy. Arrange prompt review for bleeding after menopause, repeated bleeding after sex, a strong-smelling discharge, or new pelvic pain.
