Cramps like a period during ovulation

Ovulation pain may feel like an ache, pressure or sharper twinge low in the abdomen. It can be on one side and may last for a short time or recur around the middle of a cycle. Mild pain can occur when the follicle stretches the ovary or releases fluid. Keep track of timing, side, bleeding, discharge, bowel symptoms and whether the pain follows a familiar pattern.

Pain that is new, worsening, very strong or unrelated to ovulation may have another cause, such as endometriosis, adenomyosis, an ovarian endometrioma, fibroids or pelvic inflammatory disease. A symptom alone cannot identify the cause. Read more about [Painful Periods](/symptoms/painful-periods/).

Rest, gentle movement and a warm pack over the lower abdomen may ease mild cramps. If you can take them safely, a pharmacist or clinician can advise on an appropriate pain reliever. Record when symptoms begin and end, and note any cycle changes. If cramps repeatedly interrupt work, sleep or sex, arrange a gynaecology review; evaluation may include an examination and ultrasound.

A clinician can discuss options such as the combined pill or hormonal coil when cycle-related pain is troublesome, taking your health history and pregnancy plans into account.

Seek urgent care for sudden severe pain, fainting, shoulder-tip pain, heavy bleeding, fever, repeated vomiting or a positive pregnancy test with abdominal pain. Arrange prompt medical advice for pain that persists, returns frequently, comes with unusual discharge or bleeding, or is tender with sex or urination.
