Coughing up blood when smoking cigarettes

Smoke inflames and dries the airway lining, which can make coughing forceful and mucus blood-streaked. Smoking can also contribute to persistent airway disease and infections. Bronchiectasis, pneumonia, pulmonary embolism, tuberculosis, and lung cancer are conditions a clinician may evaluate when blood is coughed up. This symptom does not prove lung cancer, and a smoker should not assume it is merely irritation. The duration of smoking, amount smoked, other symptoms, and medication history all affect the assessment.

Arrange prompt medical assessment rather than waiting to see whether it disappears. Stop smoking while you seek care and avoid strenuous activity. Bring a list of medicines, including blood thinners, and describe the blood’s colour and amount. Mention a persistent cough, hoarse voice, fever, night sweats, weight change, chest discomfort, or reduced exercise tolerance. A clinician can decide whether examination, imaging, or other tests are appropriate.

Call emergency services for heavy or repeated bleeding, difficulty breathing, chest pain, faintness, confusion, or sudden severe weakness. Obtain same-day care for fever, worsening cough, or feeling significantly unwell. Do not drive if you are breathless or dizzy.
