Coughing up blood from a sore throat

Coughing up blood with a sore throat may come from irritated throat tissue, but the source should be checked if it recurs. Dryness, forceful coughing, inflamed tonsils, nose drainage or bleeding gums can make a small streak visible; blood coughed from the chest needs a different assessment.

Notice whether the blood appears after throat clearing, brushing your teeth or a nosebleed, or is mixed into phlegm from the chest. Pneumonia may cause fever, chest discomfort and blood-stained sputum. Bronchiectasis can cause repeated mucus and airway bleeding. Tuberculosis and lung cancer may be investigated when the cough persists or other features raise concern; a sore throat with blood does not diagnose either condition. Pulmonary embolism is an urgent possibility when bleeding is paired with sudden breathlessness or chest pain.

Arrange medical advice if the blood returns, the sore throat is not settling, or you cannot tell whether it came from the throat or chest. Note the amount, colour, timing and triggers, and mention smoking, anticoagulants, nosebleeds and gum problems. Sip fluids if swallowing allows, avoid smoke and do not scrape or pick at irritated tissue. A clinician can inspect the mouth and throat and assess your lungs.

Seek emergency help for heavy or ongoing bleeding, difficulty breathing, severe chest pain, faintness, confusion, blue-grey lips or inability to swallow saliva. Emergency assessment is also needed if a sudden swollen painful leg accompanies chest symptoms. These signs can indicate an airway, breathing or circulation problem.

Ask whether the blood appears to be from the throat, nose or lower airways, and whether you need a chest examination or further tests. Clarify which changes require emergency care and how to ease throat irritation safely.
