Coughing up blood with fever

Coughing up blood with fever needs prompt medical assessment because infection and serious lung problems can look similar at first. Blood-streaked mucus may occur when inflamed airways are irritated by coughing, but fever adds information that should be evaluated rather than watched at home indefinitely.

Pneumonia can present with fever, cough, chest discomfort and blood-stained sputum. Bronchiectasis can flare with increased mucus and airway bleeding. Tuberculosis may be considered when a cough persists with fever, night sweats or weight loss. Pulmonary embolism can cause coughing up blood with sudden breathlessness or chest pain, even though fever is not its defining feature. Lung cancer is another possible diagnosis in some people, but blood in sputum does not mean cancer is present. Examination is needed to separate these possibilities.

Contact a clinician or urgent care service promptly and explain the fever, the blood, how long both have lasted and any breathing symptoms. Drink fluids if you can, rest, avoid smoking and record your temperature and the appearance of the sputum. Do not use leftover antibiotics or stop prescribed treatment without advice. A clinician may listen to your lungs and arrange imaging or other tests.

Get emergency help for difficult breathing, severe or worsening chest pain, heavy bleeding, faintness, confusion, blue-grey lips or inability to keep fluids down. Urgent care is also needed if you are becoming rapidly weaker or have a new painful swollen leg with chest symptoms.

Ask whether the pattern fits an infection, whether you need chest imaging or sputum testing, and which treatment is safe with your current medicines. Ask when fever or blood should be considered a reason to return urgently.
