Coughing Up a Mucus Ball with Blood: What Should You Know?

Thick mucus can collect when the airways are inflamed, dehydrated or not clearing secretions well. Forceful coughing may irritate small surface blood vessels. Pneumonia and bronchiectasis can produce mucus mixed with blood, while a pulmonary embolism or another lung condition needs consideration when there is sudden breathlessness or chest pain. The mucus appearance cannot distinguish these causes on its own.

A lump may also contain blood from the nose, gums or throat rather than deep in the lungs. Tell the clinician whether the material came after a deep cough, throat clearing, vomiting or a nosebleed.

Arrange a prompt medical review, particularly if this is the first episode or the blood returns. Sit upright, rest and drink fluids if you can do so safely. Record the colour and amount, associated fever or pain, and any medicines that affect clotting. Avoid smoking and strenuous exercise until you have advice. Do not rely on leftover antibiotics.

Get emergency help for ongoing or heavy bleeding, difficulty breathing, chest pain, faintness, a racing heartbeat, confusion or blue lips. Sudden symptoms after prolonged immobility or travel also need urgent attention. Children who cough blood and appear unwell should be assessed without delay.

Ask whether the blood is likely from the lungs, whether you need an examination or chest imaging, and how to manage thick secretions safely. Explain smoking or vaping exposure, recent infection, lung history, travel, immobility and medicines such as anticoagulants.
