The cough reflex explained

Short answer

The cough reflex is a protective response that clears irritants from the airways, although infection or sensitivity can make it overactive.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Purpose
The reflex helps move mucus and irritants out of the airways.
Common triggers
Cold air, talking, smoke, fragrance and throat irritation.

How the reflex works

Sensors in the throat and breathing tubes detect mucus, particles, fluid or chemical irritation. Nerves carry that signal to the brain, which coordinates a deep breath followed by a forceful burst of air. This can move material towards the mouth. After a cold, the sensors may remain sensitive, so speaking, laughing, cold air or perfume can trigger coughing. Asthma, Acid Reflux, Allergic Rhinitis and other airway conditions can also stimulate the reflex.

Reducing unnecessary triggers

Sip water, avoid smoke and strong fragrances, and use a calm nasal inhale followed by a slow exhale when a tickle starts. Treating nasal congestion or reflux may help when those problems are contributing, but the right treatment depends on the cause. Do not repeatedly force a cough to clear a dry tickle, because this can further irritate the throat. Write down when episodes happen and what was present just before them.

When the reflex needs review

Book an assessment for a cough that persists, interrupts sleep, limits activity, or comes with wheeze, hoarseness, swallowing difficulty, fever or chest discomfort. Get urgent help for severe breathlessness, blue lips, confusion, fainting or coughing blood. A clinician may examine your nose, throat and chest and decide whether breathing tests or treatment for an underlying trigger is appropriate.

Questions for your doctor

What triggers seem most relevant in my case? Could reflux, nasal drainage or asthma be involved? How can I interrupt the cough safely? Do I need tests, and what warning signs should I watch for?

Questions people ask

Can the cough reflex become sensitive after an infection?

Yes. Inflamed airway nerves can react to mild stimuli for a period after respiratory illness.

Does suppressing every cough cause harm?

A dry, repetitive cough may be calmed, but mucus-producing coughs can serve a clearing function; seek advice for persistent symptoms.
