Combined pill risks and warning signs

Oestrogen-containing pills are not suitable for everyone. A clinician weighs potential harms against contraceptive needs and possible benefits such as lighter, more regular, or less painful bleeding.

Bleeding between scheduled bleeds, nausea, breast tenderness, headache, dizziness, or mood changes may appear after starting. These symptoms are different from warning signs of a clot or vascular event, and persistent or disruptive effects can be reviewed without assuming every combined pill will feel the same.

Serious concerns include clots in a leg or lung, stroke, heart attack, severe liver problems, and raised blood pressure. Hormonal contraception also changes the risk profile of some cancers in different directions, so a personal or family cancer history belongs in the prescribing discussion.

Before starting or renewing, disclose smoking, migraine with aura, blood-clot history, high blood pressure, heart disease, diabetes complications, liver disease, breast cancer, recent childbirth, reduced mobility, and planned surgery. Include family history and every medicine or herbal product you use.

Attend blood-pressure and contraception reviews, read the specific leaflet supplied with your formulation, and avoid sharing tablets. If your health changes, arrange reassessment instead of waiting until the prescription expires. Condoms add sexually transmitted infection protection, which the pill does not provide.

Get emergency help for sudden shortness of breath, unexplained chest pain, coughing blood, a fast or irregular heartbeat with collapse, or swelling, warmth, colour change, and pain in one leg. These symptoms can occur with a clot in the lungs or a deep leg vein.

Sudden facial weakness, arm or leg weakness, speech difficulty, confusion, vision loss, or a severe unfamiliar headache also warrants emergency assessment. Prompt care is driven by the symptom itself; do not wait to see whether it settles.

Seek rapid clinical advice for new migraine aura, yellow skin or eyes, severe upper abdominal pain, a breast lump, or a major rise in blood pressure. Tell the treating team which pill you use and when you last took it.

Ask which parts of your history affect risk, whether an oestrogen-free method would suit you better, what symptoms require stopping or urgent assessment, and how often your blood pressure and prescription need review. If surgery or prolonged immobility is expected, request a contraception plan in advance.
