Combined oral contraceptive pill (COCP): a practical guide

It prevents pregnancy mainly by stopping ovulation, with additional effects on cervical mucus and the womb lining. Reliable use depends on taking active tablets as directed and managing any interruption promptly.

The COCP can produce planned withdrawal bleeding and may reduce menstrual flow or cramps. A clinician may also consider it for symptoms associated with endometriosis or polycystic ovary syndrome. It does not remove the underlying condition and does not protect against sexually transmitted infections.

Active tablets deliver hormones; any inactive tablets contain no contraceptive hormone. Brands differ, so the colour and sequence in one pack should never be assumed to match another. Keep the patient leaflet until the pack is finished.

A prescriber should review blood pressure and factors that affect estrogen safety, including migraine with aura, clot history, smoking, cardiovascular or liver disease, breast cancer, childbirth, breastfeeding and upcoming surgery or immobility. Share every medicine and supplement you take.

Take the tablets in pack order at a consistent time. After a missed active pill, vomiting or severe diarrhoea, read the product-specific instructions immediately. Depending on the interruption, you may need condoms, a revised pack schedule or advice about emergency contraception. A pharmacist can help interpret the leaflet on the same day.

If you want to stop, plan alternative contraception before the next sexual contact that could result in pregnancy. Fertility can return without waiting for cycles to become regular.

Get emergency help for sudden breathlessness, chest pain, coughing blood, a painful swollen leg, sudden weakness or numbness, difficulty speaking, sudden visual loss, collapse, or a severe unfamiliar headache. Do not take another dose while waiting for individualized medical instructions if a serious reaction is suspected.

Seek a timely contraceptive review if aura appears for the first time, blood pressure rises, side effects remain disruptive, bleeding is concerning, or pill-taking errors are frequent. A method that does not require a daily action may fit better if missed tablets are a recurring issue.

Ask why this formulation was selected, which tablets are active, what exact missed-dose rules apply, and whether your other medicines alter protection. Discuss when to check blood pressure, what bleeding pattern to expect, and which alternative would suit you if estrogen is not advised.
