Clinical manifestation of pneumonia: what should you know?

Short answer
Pneumonia is a lung infection that may cause cough, fever, phlegm, chest discomfort and breathing difficulty.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Lung features
- Cough, phlegm, breathlessness or pain with breathing
- Possible causes
- Viral, bacterial or other respiratory infection
How pneumonia may present
Pneumonia can develop suddenly or follow a respiratory illness. Common features include cough, fever, chills, tiredness, shortness of breath and coughing up phlegm. Some people have chest pain that worsens with breathing or coughing. Older adults and people with weakened immunity may show less typical signs, such as new confusion, unusual weakness or reduced appetite.
Viruses, bacteria and other organisms can cause pneumonia. Influenza and COVID-19 can affect the lungs and may be associated with pneumonia. A clinician may listen to the chest, check oxygen levels and decide whether imaging or other tests are needed. The cause and severity guide treatment.
Practical steps
Arrange medical assessment if cough, fever or breathlessness is persistent, worsening or limiting normal activity. Rest, drink fluids if appropriate and avoid smoking. Take prescribed treatment exactly as directed; antibiotics do not treat viral infections unless a clinician identifies a bacterial reason to use them.
People at higher risk because of age, pregnancy, chronic lung or heart disease, or reduced immunity should seek advice earlier. Keep track of breathing changes, fluid intake and alertness so you can describe them accurately.
Urgent warning signs
Seek emergency help for severe breathlessness, blue or grey lips, chest pain that is intense or persistent, new confusion, fainting, inability to stay awake or inability to drink. A rapidly worsening infection with very low urine output, cold clammy skin or marked weakness also needs urgent assessment because sepsis can develop.
Questions for your appointment
Ask what is most likely causing the infection, whether you need a chest image or oxygen check, which treatment is appropriate, and what change should prompt urgent review. Confirm when you can safely resume work or exercise.
