Why chills can happen without fever

Shivering is a way the body produces heat, so it can follow being cold, exhaustion or a strong emotional response. Some people feel shaky with low blood sugar, particularly after missing meals or taking diabetes medicine. Infection remains possible: influenza, a urinary tract infection, kidney infection, pneumonia, malaria and dengue fever can begin with chills or other symptoms before a fever is recorded. A single normal temperature does not rule out illness, especially after taking fever medicine. Note the timing, any exposure to mosquitoes or travel, urinary symptoms, cough, pain and changes in alertness.

What to do now

Move to a comfortable warm environment, add a light layer and drink fluids if you can. Recheck your temperature later with the same thermometer and method. Eat a small balanced snack if you may have missed a meal, unless a clinician has told you to restrict food. Review any new medicine with a pharmacist or doctor rather than stopping prescribed treatment on your own. Record associated symptoms and their onset; this helps distinguish a brief response from an illness that is developing.

When to seek medical care

Seek emergency care for confusion, fainting, severe breathing difficulty, blue or grey lips, severe weakness, a seizure, or rapidly worsening illness. Arrange prompt assessment for chills with a stiff neck, severe headache, chest pain, persistent vomiting, new rash, painful urination, flank pain, cough with breathlessness, or recent travel to a malaria area. Sepsis can develop with serious infection; feeling unusually unwell, fast breathing or marked drowsiness needs urgent attention even without a measured fever. People who are pregnant, immunocompromised or very young should seek advice early.
