Why chills may occur without a fever

A thermometer can be normal while your body feels cold or shakes. Cold surroundings, wet clothing, tiredness, anxiety, low blood sugar, or a medicine reaction may explain a short episode. Infection can also begin before a fever appears, or a fever may have settled after medicine. Influenza, urinary tract infection, kidney infection, pneumonia, malaria, and dengue fever can present with chills as part of a wider illness; chills alone cannot identify one of these conditions.

Consider what else is happening. Cough, sore throat, or body aches suggest a respiratory illness; urinary burning, back pain, or nausea deserves medical attention. Diabetes, immune suppression, and recent travel change how quickly you should ask for advice.

How to monitor yourself

Move to a comfortable environment, change out of damp clothes, and use a light blanket if needed. Check your temperature with a reliable thermometer and repeat it if the episode returns. Drink normally and eat a snack if you may have gone without food, particularly if you use diabetes medication. Write down triggers, medicines, duration, and associated symptoms. Do not start antibiotics without a clinician’s advice. If episodes recur, last beyond a brief period, or interfere with daily activities, arrange a primary-care review.

Warning signs that need assessment

Get emergency help for confusion, collapse, severe breathlessness, chest pressure, blue or grey skin, or difficulty staying awake. Seek prompt care for severe pain, repeated vomiting, a stiff neck, a new widespread rash, very little urine, or rapidly worsening weakness. Contact a clinician promptly after travel to a malaria-risk area, even without fever. A normal temperature does not rule out a significant illness, especially in older adults or people with weakened immunity.
