Causes of deep dyspareunia

It differs from pain mainly at the vaginal opening, although both types can occur together. The location, positions that trigger pain, cycle timing, and symptoms outside sex help a clinician investigate the source.

Endometriosis and ovarian endometriomas can present with deep pain during sex, often alongside painful periods or pelvic pain. Pelvic inflammatory disease can present with deep dyspareunia as well as discharge, bleeding, fever, or lower abdominal pain. Ovarian cysts, uterine conditions, pelvic scar tissue, bladder or bowel problems, and overactive pelvic-floor muscles are other possibilities.

Genitourinary syndrome of menopause can cause dryness and fragile tissue that make penetration painful. Vulvodynia and lichen sclerosus more often affect the vulva or entrance, but a person may describe mixed or poorly localised pain. Pain location alone cannot confirm a diagnosis.

Stop any activity that hurts; you do not need to push through pain. Try positions that let you control depth and pace, allow more time for arousal, and use a suitable lubricant if dryness contributes. Arrange an appointment when pain recurs or affects intimacy. A clinician may ask about periods, discharge, bleeding, contraception, pregnancy possibility, menopause symptoms, surgery, bladder or bowel changes, and trauma, then offer an examination or imaging with your consent.

Seek urgent assessment for sudden severe pelvic pain, faintness, heavy bleeding, vomiting, fever, or pain with a positive pregnancy test. Prompt review is also needed for new foul-smelling discharge, worsening lower abdominal pain, sores, or bleeding after sex. These signs guide urgency; they do not establish the cause.

Ask whether the pattern suggests pelvic-floor involvement, infection, endometriosis, or another organ; which parts of an examination are optional; and whether ultrasound or infection tests could help. Discuss treatments matched to the suspected source, fertility goals, and any menopause-related dryness.
