C3-C4 ACDF surgery: what patients should know

C3 and C4 are adjacent vertebrae in the upper part of the cervical spine. ACDF stands for anterior cervical discectomy and fusion: the surgeon reaches the spine through the front of the neck, removes the disc and compressive material, then places a spacer or bone graft in the disc space.

A plate and screws may add stability while fusion develops. Because the approach passes near the swallowing tube and voice structures, temporary throat soreness, swallowing discomfort, or voice change can occur. Fusion stops movement at the treated segment, although the rest of the neck continues to move.

Give the surgical team a complete medication and supplement list and follow its specific instructions about eating, drinking, and medicines before the operation. Arrange transport home and practical help for the early recovery period. If you smoke or use nicotine, discuss stopping because nicotine can interfere with bone healing.

After surgery, follow the wound, collar, lifting, driving, and activity instructions supplied by your team. Walking is commonly encouraged in short, comfortable intervals. Take prescribed medicines only as directed, keep follow-up appointments, and confirm when physiotherapy or neck exercises should begin rather than starting them independently.

Get emergency help for trouble breathing, rapidly increasing neck swelling, inability to swallow saliva, sudden new arm or leg weakness, loss of bladder or bowel control, or severe chest pain. These symptoms require assessment without waiting for a routine appointment.

Contact the surgical team promptly for fever, spreading redness, drainage from the incision, worsening pain that is not controlled by the agreed plan, persistent vomiting, or new numbness. Mild throat discomfort can occur after the front-of-neck approach, but worsening swallowing or voice symptoms should be reported.

Ask which finding at C3-C4 matches your symptoms, whether the main goal is to protect the spinal cord or relieve a nerve, and what improvement is realistic for pain, sensation, strength, or walking. Clarify which graft or spacer and fixation will be used, whether a collar is planned, and what restrictions apply to work and driving.
