Bulky uterus with focal adenomyosis: what the finding means

This is a description of the womb's appearance on imaging, not a measure of how severe your symptoms are. Adenomyosis is non-cancerous, and some people have no symptoms. Others experience heavy or painful periods, pelvic aching, pressure, bloating or pain during sex.

“Bulky” means the uterus looks larger or fuller than expected. “Focal” means the changes are mainly in a particular part of the uterine muscle, while adenomyosis describes tissue similar to the womb lining growing within that muscle. The report may also describe wall thickening or a less clearly defined area.

A scan result is interpreted alongside your bleeding pattern, pain, examination and pregnancy plans. Fibroids and other pelvic conditions can produce overlapping symptoms or imaging appearances, so your clinician may review the images or arrange further assessment if the diagnosis is uncertain.

Book a routine gynaecology or primary-care appointment if periods are becoming heavier, more painful or disruptive. Keep a cycle record noting bleeding, clots, pain, medicines and missed activities. Your clinician may check for anaemia when bleeding is heavy and may use ultrasound or MRI findings to clarify the cause.

Treatment is based on symptoms rather than uterus size alone. Options can include pain relief, medicine to reduce bleeding, or hormonal treatment such as a hormonal coil. Procedures may be discussed when symptoms remain troublesome; whether endometrial ablation, uterine artery embolisation or hysterectomy is suitable depends on the pattern of disease, previous treatment and whether you wish to carry a pregnancy.

Arrange prompt medical assessment if pain is severe or worsening despite pain relief, bleeding is much heavier than usual, or you feel unusually weak, breathless, dizzy or faint. New bleeding between periods or after sex also needs evaluation rather than being attributed automatically to adenomyosis.

Go to an emergency department for severe or rapidly worsening pelvic pain, fainting, difficulty breathing or very heavy vaginal bleeding. Seek urgent assessment if you may be pregnant and have pelvic pain or bleeding, because pregnancy-related causes require separate evaluation.

Ask which features on the scan support focal adenomyosis, whether anything else was seen, and whether your symptoms warrant blood tests or further imaging. Discuss which options fit your main concern—bleeding, pain, pressure or fertility—and how each could affect contraception, future pregnancy and the need for follow-up.
