Bronchitis and coughing up blood: what to know

Bronchitis can irritate the airways enough to cause blood-streaked mucus, but coughing up blood needs medical assessment because infections, clots, and other lung conditions can also cause it. The blood may appear as pink, red, or rust-coloured streaks mixed with phlegm. Even when bronchitis is present, the symptom should be described to a clinician rather than attributed to bronchitis without an examination.

Bronchitis is inflammation of the tubes that carry air into the lungs. Repeated forceful coughing and inflamed airway lining can make small surface blood vessels leak, producing streaks in mucus. Blood can also come from the nose, mouth, or throat and then be noticed during a cough, so its source is not always the lungs.

A chest infection such as Pneumonia may cause cough, fever, chest discomfort, and blood-stained phlegm. Longer-lasting or recurrent bleeding can have other explanations, including Bronchiectasis, Tuberculosis, Lung Cancer, or Pulmonary Embolism. These links describe conditions that a clinician may consider; seeing a blood streak does not identify which condition is present.

Arrange prompt medical advice, especially if the bleeding is new, returns, or occurs with breathlessness, fever, chest pain, dizziness, night sweats, or unexplained weight loss. Note when it started, whether the blood is mixed with mucus or appears alone, how much you saw, and whether you take medicines that affect bleeding. Do not smoke, and avoid strenuous activity until you have been assessed if coughing triggers more blood.

A clinician may listen to your chest and ask about recent infections, smoking, travel, tuberculosis exposure, medicines, and previous lung problems. Depending on the findings, they may arrange tests such as chest imaging or blood tests. Treatment depends on the cause; antibiotics do not treat every episode of bronchitis.

Seek emergency help if you are coughing up more than small streaks, the bleeding is ongoing, or you have trouble breathing, severe chest or upper-back pain, faintness, confusion, blue lips, or a very fast heartbeat. These features can signal a serious problem such as a major airway bleed or Pulmonary Embolism. Do not drive yourself if you feel faint or short of breath.

If the blood has stopped and you feel stable, contact a doctor promptly for advice. Bring a list of your medicines, including blood thinners, and mention any recent procedure or injury.

Ask: Could the blood be coming from my airways or elsewhere? What examination or imaging do I need? Which warning signs mean I should go to emergency care? Should I change any medicine that affects bleeding? What can I safely use for the cough, and when should persistent or returning blood be reviewed?
