Blood tests for hair loss: when they help

Short answer
Blood tests can help find selected medical contributors to hair loss, but the right tests depend on the shedding pattern, symptoms, diet, medicines and examination.

The short answer
They are especially useful when diffuse shedding or wider health symptoms raise questions about anaemia, iron stores, thyroid function or nutritional status. They cannot by themselves identify every scalp disorder or explain all pattern hair loss.
Why the pattern comes before the panel
Telogen effluvium commonly presents with increased shedding across the scalp and may follow illness, major physical stress, dietary restriction or another body-wide trigger. Androgenetic hair loss commonly presents as patterned thinning, while alopecia areata often presents as distinct smooth patches. Scalp ringworm can present with scale, broken hairs and inflamed areas. Each pattern points toward a different investigation.
A clinician may choose a blood count, iron measures, thyroid measures or selected nutrient tests when relevant. Results need clinical interpretation: a value outside a laboratory range may be unrelated, and a normal result does not exclude traction, infection, inflammation, genetic pattern loss or hair-shaft damage.
Make the appointment more informative
Track when the loss started, where it is most visible, whether hairs shed from the root or break, and any scalp itch, soreness or scale. Mention recent illness, childbirth, menstrual changes, weight change, restrictive eating, tight hairstyles and all medicines or supplements. Clear photographs of the parting, temples and crown can reveal progression.
Avoid starting iron, biotin or hormone products before assessment unless they were prescribed for another reason. Supplements can cause adverse effects, interact with medicines or interfere with laboratory testing. Gentle hair care and reduced traction are reasonable while the cause is evaluated.
Reasons to arrange care soon
Seek timely clinical review for abrupt shedding, new bald patches, eyebrow or eyelash loss, scalp pain, pus, marked scale, scarring or shiny areas without follicle openings. Medical review is also appropriate when hair loss accompanies pronounced fatigue, faintness, palpitations, unexplained weight change or temperature intolerance. Painful spreading redness or fever needs urgent assessment.
What to ask about the work-up
Ask which diagnosis the hair pattern suggests, what each proposed blood test is intended to detect, and whether the result would alter care. If tests are normal, clarify whether dermoscopy, fungal sampling, a pull test or scalp biopsy would answer the remaining question.
