Blood coughed up with phlegm: what to know

A small streak can follow forceful coughing, but blood in phlegm can also occur with Pneumonia, Bronchiectasis, or Tuberculosis. A Pulmonary Embolism may cause sudden breathlessness or chest pain with coughing blood. Lung Cancer is another possible cause, particularly when bleeding repeats or a cough changes, although this symptom alone cannot identify cancer. Blood from the nose or mouth can mix with saliva and be mistaken for coughed-up phlegm. The amount, colour, timing, and accompanying symptoms help guide assessment.

Contact a healthcare professional for new, repeated, or unexplained blood in phlegm. Record the amount and appearance, and mention fever, breathlessness, chest pain, recent infection, travel, surgery, immobility, smoking, or medicines that affect clotting. Avoid smoking and strenuous activity until advised. Do not self-treat with leftover antibiotics. If a known lung condition is present, follow its written action plan while arranging review.

Call emergency services for more than a small amount of blood, bleeding that continues, severe difficulty breathing, sudden chest pain, faintness, confusion, or blue lips. These signs can indicate a serious lung or circulation problem. Same-day advice is appropriate for fever with worsening cough, repeated blood, or reduced breathing tolerance, even when the blood is only lightly streaked.
