Black stool after a C-section: medicines, bleeding, and care

Check the texture and accompanying symptoms as well as the colour. Iron commonly darkens formed stool, whereas digested blood often produces stool that is jet black, sticky, difficult to flush, and unusually strong-smelling.

Iron prescribed for anaemia is a frequent explanation and may also cause constipation, nausea, or abdominal discomfort. Some dark foods or medicines containing bismuth can change stool colour as well.

Black tar-like stool, called melaena, can occur when bleeding higher in the digestive tract is altered during digestion. Anti-inflammatory pain medicines can irritate the stomach or contribute to an ulcer, particularly in someone with previous ulcer disease or when combined with blood-thinning medicine. A C-section itself does not make tar-like stool an expected recovery symptom.

Review your discharge list for iron, bismuth, anti-inflammatory pain relief, aspirin, or anticoagulants. Contact the maternity team or prescribing clinician if the stool is new and black. Describe whether it is formed or tarry and mention stomach pain, vomiting, weakness, dizziness, or breathlessness.

Do not stop a prescribed anticoagulant or other essential medicine on your own. A clinician can weigh bleeding concerns against the reason it was prescribed. Keep hydrated and follow post-operative bowel advice, but do not treat suspected bleeding with a laxative.

Seek urgent assessment for black, sticky, tar-like stool, especially if it recurs. Call emergency services for fainting, confusion, severe weakness, breathing difficulty, racing heartbeat, vomiting blood or coffee-ground material, or severe abdominal pain.

Also obtain urgent postpartum care for heavy vaginal bleeding, worsening wound pain, spreading redness, pus, fever, chest pain, or a painful swollen leg. These symptoms may be separate from the stool change but still require prompt evaluation after surgery.

Ask whether iron fully explains the appearance, whether any pain medicine could irritate your stomach, and whether a blood count or digestive evaluation is needed. Bring the medicine list and state when each drug was started, including over-the-counter products and supplements.
