What should you know about black milia under eye?

Typical under-eye milia are white or cream, so a black bump should be identified by a clinician before removal. Darkness may come from a tiny crust, an open clogged pore, pigment or another type of lesion. Because eyelid skin is thin and close to the eye, self-treatment carries added risk.

Milia form when keratin becomes trapped in a small cyst under the surface. They feel firm and usually lack the opening seen in a blackhead. Shadows along the eyelid and natural differences in skin tone can alter how a pale bump appears, but a dark centre or uniform black colour is not a reliable sign of milia.

An eye-care clinician or dermatologist can inspect the surface, borders and pigment while protecting the eye. The exact position matters: a bump on the lash line or inner eyelid may require assessment by an ophthalmologist rather than routine facial treatment.

Avoid needles, comedone tools, scrubs and acids near the bump. Do not place a retinoid on the eyelid unless a prescriber has specified exactly where and how to use it; migration into the eye can cause marked irritation. Keep cosmetics off an inflamed site and cleanse gently without rubbing.

Confirmed milia can sometimes be left alone. If removal is wanted, a trained clinician may open the surface and lift out the keratin using sterile equipment and appropriate eye protection. The treatment choice should reflect how close the bump sits to the lash line and whether the diagnosis is certain.

Have a dark under-eye bump examined if it is new, enlarging, irregular, bleeding, persistently crusted or changing colour. Seek urgent assessment for eye pain, altered vision, marked eyelid swelling, spreading redness, fever, pus or difficulty opening the eye. These features call for timely care whether the original bump is a cyst, a blocked gland, an infection or another lesion.

Ask whether the bump has the closed structure of milia or needs closer examination for another cause. Clarify who should remove it, how the eye will be protected, whether tissue testing is appropriate and which products must stay away from the eyelid. Also ask what swelling, crusting or colour change is expected after a procedure.
