What should you know about biphasic wheezing?

Air moving through narrowed tubes can create wheezing. Hearing it in both directions may mean airflow is restricted throughout the breath, although the sound alone cannot show where the problem is. Asthma, COPD, Bronchiolitis, Acute Bronchitis, and Allergic Rhinitis can present with wheezing. Anaphylaxis can cause sudden airway symptoms and requires emergency action; wheezing itself does not establish any one diagnosis.

Move away from smoke, strong scents, or a suspected allergen and sit upright if that feels easier. Use a prescribed reliever exactly as directed. Do not borrow another person’s inhaler or delay care to try home remedies. Note when the sound began, whether it follows exercise or exposure, and whether cough, chest tightness, fever, or swelling is present. Call emergency services for severe breathlessness, inability to speak in full sentences, swelling of the lips or tongue, faintness, blue or grey skin, or rapidly worsening wheezing. Get prompt medical advice for a new sound, wheezing in a young child, symptoms after choking, or wheezing that does not settle with prescribed treatment. These signs concern airflow and oxygenation, regardless of how common a particular cause may be.
