Back-of-Neck Pain with Headache

Short answer
Neck pain and headache can occur together when neck muscles or joints are irritated, but sudden or neurological symptoms require urgent assessment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Observe
- Note whether turning or holding the neck still changes the headache.
- Avoid
- Do not use forceful neck manipulation when symptoms are new or unexplained.
- Emergency
- Sudden severe headache with a neurological change needs emergency help.
How to interpret these symptoms
The relationship between head and neck symptoms offers useful clues: note which started first, whether neck movement changes the headache, and whether injury, fever, visual symptoms, numbness, weakness or vomiting is present.
How different patterns are assessed
A Muscle Strain or Myofascial Pain Syndrome can present with local tenderness and pain referred toward the head. Text Neck may present with discomfort related to sustained device posture. Cervical Spondylosis can present with neck stiffness and pain, while Cervical Radiculopathy or a Herniated Disc may produce arm pain, tingling, numbness or weakness. Whiplash may present with neck pain and headache after rapid head movement.
These overlapping features do not establish a diagnosis. The examination checks movement, tenderness, limb strength, sensation, reflexes and coordination when appropriate.
Practical next steps
If symptoms are mild and no warning signs are present, vary your posture, reduce aggravating screen time, and move the neck gently within comfort. A wrapped cold pack or warmth may provide temporary relief. Avoid forceful manipulation, especially after trauma or when the cause is unclear.
Schedule an assessment if headaches are new and recurrent, the neck remains restricted, pain follows an injury, or arm symptoms develop. Keep a note of triggers, headache location, associated symptoms and any medicines taken.
Urgent and emergency signs
Call emergency services for an abrupt, exceptionally severe headache; new weakness or numbness; facial droop; trouble speaking; collapse; confusion; seizure; loss of coordination; or sudden visual loss. Neck pain and headache after a significant collision, fall or blow also needs prompt assessment.
Seek urgent medical care for fever with a stiff neck, a new rash, repeated vomiting, marked drowsiness, or strong sensitivity to light. Do not drive yourself if you feel faint, confused, weak or unable to see clearly.
Questions to bring to the consultation
Ask whether the headache appears related to the neck, whether neurological testing is needed, which movements are suitable, and what change would require urgent reassessment. Report any recent infection, trauma, pregnancy, new medicine or previous headache pattern.

Questions people ask
Can tense neck muscles cause a headache?
Tight or irritated neck muscles can refer discomfort toward the back or sides of the head. Other causes can feel similar, so the broader symptom pattern matters.
Can posture contribute to both symptoms?
Holding the head forward or staying in one position can load neck muscles and joints. Changing position often and adjusting the screen may reduce this trigger.
Should I exercise through neck pain and headache?
Choose easy movement that does not intensify either symptom. Stop and seek advice if activity brings dizziness, visual change, weakness, numbness or a sudden increase in headache.
