The short answer
It can accompany cough, chest tightness, or shortness of breath. The sound alone does not identify the cause; age, speed of onset, triggers, infection symptoms, and breathing effort guide the assessment.
How narrowed airways create the sound
Airways may narrow because their muscles tighten, their lining swells, or mucus collects inside them. Asthma commonly presents with episodes of wheezing, while viral bronchitis, bronchiolitis in young children, COPD, and allergic reactions can also present this way.
A noise from the throat can be mistaken for wheeze. A harsh sound mainly when breathing in, particularly with difficulty swallowing or voice change, may arise from the upper airway and needs prompt assessment.
What to do when wheezing starts
Sit upright, move away from smoke or a known irritant, and remain with anyone whose breathing is affected. If you have a prescribed reliever inhaler and a personal action plan, use them exactly as directed.
Do not use another person's inhaler or delay care while trying steam or home remedies. A clinician may check oxygen level, listen to the chest, assess airflow, and choose treatment according to the cause. Nebulised medicine is useful in selected situations, not automatically for every wheeze.
When wheezing is an emergency
Call emergency services for severe breathing difficulty, blue or grey lips, collapse, confusion, inability to speak comfortably, marked chest pulling, or rapid worsening. Sudden wheeze with swelling of the lips or tongue, widespread hives, throat tightness, or faintness may be anaphylaxis and needs emergency treatment.
Seek same-day medical advice for a first episode, wheezing in a baby, symptoms with fever or chest pain, or an episode that does not settle with prescribed treatment. Recurrent night-time or exercise-related wheeze deserves a planned assessment even when breathing later returns to normal.

Does wheezing always mean asthma?
No. Asthma often presents with wheezing, but infections, COPD, allergic reactions, inhaled irritants, and other airway problems can also cause it. The surrounding symptoms and examination help identify the cause.
Can wheezing happen without feeling short of breath?
Yes. Mild airway narrowing may produce an audible sound before breathlessness is prominent. New or recurring wheeze still warrants assessment, especially if it interrupts sleep or activity.
